Important instructions: Please make sure you read and complete both sections below before continuing with the application process.

1. Please read and agree to three documents of consent.

2. Please gather and prepare information about health condition of your child(s) for the Health form:

Immunization history records

Names of the medications, if your child taking any

Health conditions of your child(s) that school doctor and personnel should be aware, such as allergies, hearing, vision, seizure and other problems

3. Please prepare files (scans) of following documents, you will need them on "Upload documents" step:

Your child's recent photo (JEPEG, GIF, PNG, BMP format)

Children's Passports or Birth Certificates (copy)

Father's passport scan (JEPEG, GIF, PNG, BMP or PDF format)

Father's recent photo (JEPEG, GIF, PNG, BMP format)

Mother's passport scan (JEPEG, GIF, PNG, BMP or PDF format)

Mother's recent photo (JEPEG, GIF, PNG, BMP)

Children's vaccination records scan (JEPEG, GIF, PNG, BMP)

For child applying to 3-7 year-old class, please upload the letter of attendance, if your child attended school or pre-school (JEPEG, GIF, PNG, BMP or DOC, DOCX, PDF format)

For child applying to 8-13 year-old class, please upload the school records for the last completed school year and a current year if you apply in the middle of the school year (JEPEG, GIF, PNG, BMP or DOC, DOCX, PDF format)

For a child applying to Secondary I (grade 9) and up please upload the transcript (JEPEG, GIF, PNG, BMP or DOC, DOCX, PDF format)
